Level Health offers three membership paths, each a 12-month commitment with two payment options. Individual Membership is $3,000 paid in full, or $1,750 every 6 months ($3,500 total). Family Add-On (added to an active Individual membership) is $2,000 paid in full, or $1,250 every 6 months ($2,500 total). Family Bundle covers two memberships at $5,000 paid in full, or $3,000 every 6 months ($6,000 total). Every membership includes a personalized, physician-directed protocol drawing from Biomarker-Driven Cellular Optimization or HRT as clinically indicated; quarterly physician visits; unlimited physician access via Spruce; lab-informed dosing adjustments; three private sessions with our fitness coach covering nutrition and training; year-over-year biomarker tracking; and priority scheduling. Recommended therapeutics are billed separately and shipped to your door.

Every patient's protocol is different — your physician will recommend only what's clinically appropriate for you, based on your labs, goals, and medical history. Billing recommended therapeutics separately ensures you only pay for what's recommended for your body, rather than being charged for therapies you may not need.

Level Health is a direct-pay, cash-based practice. We do not accept insurance — this is intentional. Operating outside the insurance system allows our physicians to spend more time with you, design protocols that are truly personalized, and avoid the restrictions that insurance-driven care imposes. Invoices are provided through QuickBooks.

Level Health is currently licensed to practice telemedicine in New York, Pennsylvania, and New Jersey. We serve patients residing in any of these three states through our HIPAA-compliant telemedicine platform, so wherever you are within our licensed footprint, care is only a call away.

All patients receive quarterly consultations — every 90 days — to review updated labs, assess progress, and adjust your protocol. Between visits, our care team is accessible for questions through our HIPAA-compliant platform.

Recommended therapeutics are shipped directly to your home on a schedule coordinated with your protocol.

Results vary by protocol and individual baseline, but most patients begin noticing measurable improvements in energy, focus, sleep, and body composition within 4–8 weeks of starting their protocol. Optimization is iterative — we track and adjust to keep results compounding over time.

Yes. Every protocol at Level Health is actively monitored by a licensed physician. We conduct baseline and follow-up lab work to ensure safety and efficacy. Your physician will walk you through all options, expected outcomes, and any relevant risks during your consultation.

Your Discovery Call is a complimentary 15-minute conversation with our care team. We'll learn a little about you, walk you through how our practice works, and answer any questions you have about the membership paths, therapies, or process. There is no clinical evaluation on this call and no pressure to enroll — it exists so you can decide whether Level Health is the right fit before committing.

Once you've chosen a membership path, your first clinical visit is a 60-minute Initial Consultation with Dr. Fajardo-Ahmad. Just yourself and 60 minutes — we'll cover your health history, current symptoms, goals, and lifestyle. From there, we coordinate diagnostic labs and follow up with your personalized protocol once results are in.

All memberships are a 12-month commitment and non-refundable for the term, whether paid in full at signup or on the payment plan. If you elect the payment plan, the second installment is billed automatically at the six-month mark — both installments are owed regardless of usage. Auto-renewal may be canceled up to 30 days before your term ends. In rare cases where baseline bloodwork reveals a medical contraindication to your planned protocol, a prorated refund may be issued at the physician's discretion.
